For service providers at children's mental health agencies
We have partnered with a number of children's mental health agencies in the province to recruit participants for this study. Parents and youth who have received services at these agencies are eligible to participate and are being asked to complete a questionnaire about their continuity of care.
Participants in the study also provide their consent for us to contact their primary clinician at the agency with which they are involved, to request that the clinician complete a brief measure of that client's care from their perspective.
Clinicians are faxed the consent form for their records and can complete the measure online.
All responses are confidential. Clinicians will not see their clients' responses, nor vice versa.
